Admissions Process

The Different Phases of Admission

Learn about the key steps in Mooseheart’s admission process, from initial application to becoming part of our community.

Phase 1

The initial step in seeking application to Mooseheart is to contact the Mooseheart Admissions Department and request an application packet.

The Admissions Specialist will then gather the following information to establish eligibility:

  • Family Name
  • Address and Contact Information
  • Name and Age of Children for whom you are applying
  • Health of Children
  • Any knowledge of special education needs
  • Counseling services, behaviors of concern, etc.
  • Reason you are seeking admission for the child(ren)
  • Appropriate signed release forms for child’s school & any other professional services he/she has received

Once the questionnaire and release forms are submitted to the Admissions Department, the Admissions Specialist evaluates the available data to determine whether or not the applicant should receive further consideration for Mooseheart services. After the Admissions Specialist has made the decision, the family who made the inquiry will be notified within a reasonable time.

Phase 2

The Admissions Specialist will initiate the second phase of the admission process by sending all necessary forms along with a checklist to the applicant’s parent or guardian.

Mooseheart will also request the following items:

  • Any available documentation describing the legal status of the child issued to the guardians. (i.e. court
    ordered temporary or permanent custody/guardianship records, divorce decree, etc.)
  • An original birth certificate and an original social security card. No child can be admitted to Mooseheart
    without these documents.
  • Copies of any religious certificates, particularly a baptismal certificate, if applicable
  • Current school grades
  • A recent photo of child and guardian
  • A physical examination from within the past 12 months for each child being considered

Phase 3

Once all necessary forms from Phase One and Two have been completed and returned to the Admissions Department, we will enter into the final stage of the admission procedure.

Phase Three consists of two steps:

  1. All pertinent data is summarized into a concise report with all discrepancies verified by telephone
    interview. The final report is then presented to the Admission Committee, where each case is decided on by
    its individual merits.
  2. The Admissions Specialist will contact the family with the committee’s final decision. Those families
    whose children are accepted for placement will work with the Admissions Department to schedule a date for
    the child(ren’s) intake. Children are accepted and placed throughout the year.

Application Forms

Application

All questions are to be answered by the parent/guardian. The handwritten statement of the parent/guardian(s) and their signature is to be completed in the space provided on page three. The President and Lodge Administrator of the sponsoring Moose lodge should provide statements, signatures, and the lodge’s seal in the spaces provided on page four, thereby indicating the lodge’s willingness to sponsor the child(ren).

Medical Forms

The Medical History Form is completed by the parent/guardian. The Medical Examination Form is to be completed and signed by a physician, providing the results of a current examination.

Guardianship

In the case of a child(ren) being raised by someone other than a biological parent, e.g. a grandparent or other kinship caregiver, it is critical that appropriate legal documentation be provided.

Social Security Card and Birth Certificate

Please note we must have the original birth certificates and social security cards. Copies of these documents are NOT acceptable.

Financial Responsibility

Included is a letter, which indicates the parent/guardian’s awareness that if any
type of support such as social security is being received by the parent/guardian, theses funds come to Mooseheart during the time the child is a resident at Mooseheart. Adoption assistance and child support along with social security.

How does Tuition Work?

Parents/Guardians still have a financial responsibility for having a child attend Mooseheart Child City and School.

Admissions FAQ

No, Mooseheart does not become legal guardian of its residents. Guardians
must be willing to retain their rights while their child resides at Mooseheart. It could be
compared to a child residing at a private boarding school

Most students will visit the home of their guardians for major holidays (Christmas,
Thanksgiving), spring break, and a few weeks during the summer. Students with families
who live closer may visit their guardians as much as, but no more than, 2 weekends a
month.

Mooseheart is supported by a portion of each Moose member’s dues paid to their Lodge,
along with a portion of the interest earned from the Mooseheart Endowment Fund.
